The skilled nurse will assume responsibility for notifying the attending physician of any adverse circumstances, which could prevent safe independent management in the home setting.

Assessment / Coordination of the nursing rehabilitative, social and D.M.E. needs of the patient.
Observation of the patient's clinical condition including treatments and changes in physical and emotional status.
Teaching, supervising and evaluation of the patient's and family's ability to care for the patient in the home.
